Resident Enrollment

Welcome to Lake Shore

To register, please begin by completing our Resident Enrollment Form.  Parents must supply:

  • An original birth certificate or certified copy, with raised seal, issued by the clerk of the county where your child was born or the health department of the state of birth

  • Proof of Residency

  • Your child's health immunization record; including vision and hearing screenings. Be sure to schedule immunization updates early these appointments are often limited

Class of 2032

Take your next steps toward graduation with Lake Shore Public Schools by joining the Class of 2032. Children are eligible for Kindergarten if they turn five years old on or before September 1, 2019. However, children who turn five between September 1 and December 1, 2019 are eligible to enroll in Kindergarten if their parents submit a waiver form.

Food Service Applications

Applications for the 2019-20 school year will be available after July 1, 2019 per MDE policy.  After July 1, 2019 you may go online or submit a paper copy to Food Services. A new application is needed EVERY YEAR for all students, one per family with all students names. Previous year's application is still in affect 30 business days of the new school year.
